What is Peroneal Tendonitis?
Peroneal tendonitis is a condition involving irritation and inflammation of the peroneal tendons, which run along the outer side of the lower leg and behind the bony bump of the ankle (lateral malleolus). These tendons play a key role in stabilising the foot and ankle, especially during walking, running, and side-to-side movement. Peroneal tendonitis is usually caused by overuse, repetitive ankle movements, sudden increases in activity, or ankle sprains, and is often seen in runners, dancers, or those with high-arched feet.

Typical Symptoms of Peroneal Tendonitis
How Do We Treat Peroneal Tendonitis at Our Clinic?
At Elite Physical Medicine, you’ll receive a thorough and bespoke rehabilitation programme:
Comprehensive Assessment: We perform a detailed assessment to confirm peroneal tendonitis, check for contributing factors (such as foot posture or previous injuries), and rule out other causes of outer ankle pain. A diagnostic ultrasound scan can also be performed by one of our Sonographers to give a clearer, more detailed assessment.
Education & Advice: Practical guidance is provided on footwear, training modification, pacing activity levels, and avoiding aggravating movements whilst the tendon settles.
Exercise Programme: Personalised exercises focus on gentle stretching, progressive strengthening for the peroneal muscles, and balance/proprioceptive training to restore ankle control and support tendon recovery.
Manual Therapy: Soft tissue therapy, massage, and gentle joint mobilisation are used to relieve discomfort, reduce tension, and improve flexibility in the foot and ankle.
Pain Management: Advice is given on managing pain with ice application, rest where needed, and over-the-counter pain relief.
Activity Modification: Recommendations are made for temporarily reducing or adapting high-impact activities and gradually returning to normal routines and sport as symptoms improve.
Additional Treatment Options for Peroneal Tendonitis
Referral for Imaging or Specialist Care: If symptoms persist or there’s suspicion of a tendon tear or longstanding problem, imaging (such as ultrasound or MRI) or specialist referral may be considered.
