What is Piriformis Syndrome?
Piriformis syndrome is a rare condition in which the piriformis muscle, a small muscle deep in the buttock, irritates or compresses the sciatic nerve as it passes underneath (or sometimes through) the muscle. The result is pain, tingling, or numbness in the buttock and possibly down the back of the leg, similar to sciatica. Piriformis syndrome is uncommon, accounting for a small proportion of all sciatica cases, and its diagnosis requires careful clinical assessment to exclude other, more frequent causes such as lumbar disc herniation.

How Do We Treat Piriformis Syndrome at Our Clinic?
At Elite Physical Medicine, our physiotherapists offer expert, evidence-based care targeted to this rare condition:
Comprehensive Assessment: We provide a thorough examination to confirm diagnosis and rule out other, more common causes of buttock or leg pain (such as lumbar spine issues). Assessment includes biomechanics, posture, muscle strength, and nerve tests.
Education & Advice: Information and practical advice on ergonomics, posture, and activity modification is given to help reduce muscle irritation and nerve compression.
Exercise Programme: A personalised rehabilitation plan focuses on targeted stretching for the piriformis and other hip muscles, strengthening exercises for the glutes and core, and progressive mobility work to restore normal movement and reduce nerve symptoms.
Manual Therapy: Hands-on techniques such as deep tissue massage, myofascial release, and mobilisation are used to help relieve muscle tightness, improve circulation, and reduce pressure on the sciatic nerve.
Pain Management: Advice is offered on using ice or heat, rest periods, and over-the-counter pain relief when appropriate.
Activity Modification: Temporary changes to sitting posture, sport, and daily routines may be recommended to ease symptoms and support healing.
Additional Treatment Options for Piriformis Syndrome
Referral for Further Investigation: Very rarely, if symptoms persist or diagnosis remains uncertain, further assessment such as imaging or review by a specialist may be arranged. Injection therapy or surgery is rarely indicated and considered only after thorough assessment.
